We need a form script that allows:
1. Includes polls as required fiedls
2. Basic required fields
3. Must allow indiviuals (sub-webmasters) other than the webmaster modify the form for their own independent
4. Webmaster and sub-wbmasters must be automatically notified when end users of the form do not complete the form during a predetermined time period.
5. Results of the online polls and data collected from the forms must be retained for a specified time period allowing web master and sub-webmasters to view the collected data at any time.
6. End users of the form should be automatically notified via email when they fail to complete and submit for as agreed during a predefined time period.
Additional Info (Added 2/23/2008 at 7:40 (MST))...In addition, there must be logins for three different users. 1. Webmaster
2. Sub webmaster
3. End Users
